Haemophilia Health for members

Pfizer Australia Pty Ltd (Pfizer) has developed the Haemophilia Health patient support program for Australians who have been prescribed a Pfizer haemophilia medication (Program).

Terms and Conditions of the Program

Your use of the Program website is subject to Pfizer’s Terms of Use (available at http://pfizer.com.au/sites/au/LegalDisclaimers/Pages/TermsofUse.aspx).

By signing up to the Program, you warrant that you are currently prescribed, in Australia, a haemophilia medication manufactured by Pfizer.

If you are under the age of 18 years, you acknowledge that your enrolment in the Program is subject to your parent/guardian’s permission. Before applying to enroll in the Program, you must discuss it with you parent/guardian and get their consent.

The content of the Program (including the Program website) is not intended to be a substitute for professional medical advice, diagnosis or treatment. Always consult a qualified healthcare professional with any questions you may have regarding a medical condition or your treatment.

Pfizer reserves the right to alter or modify the Program content, from time to time or to discontinue the Program at any time.

You must keep your username and password to access the Program confidential and not share your username or password with anyone else. You are responsible for all usage of the Program via your account.

Your use of the Program is at your own risk. Nothing in these Terms and Conditions excludes, restricts or modifies any consumer guarantee, right or remedy conferred on the Program user by the Australian Consumer Law or any other applicable Law that cannot be excluded, restricted or modified by agreement.  Subject to that, any condition, warranty, guarantee, right or liability which would otherwise be implied in this agreement or imposed by law is excluded.

Your use of the Program subject to these Terms and Conditions shall be governed by the laws of New South Wales, Australia.

Program Privacy Statement

We will need you to share some of your personal and sensitive information with us in order to administer the Program, provide you with the full range of support that is available under the Program and comply with reporting obligations for adverse events.

Your personal and sensitive information will be used to provide the services associated with the Program, information regarding the Program and any goods or services you elect to receive or request from Pfizer. If you are unwilling to share your personal and sensitive information with us then we will not be able to provide you with the services.

We will use and handle your information in accordance with this Program Privacy Statement, the Privacy Act 1988 (Cth) and the Pfizer Australia Privacy Policy which can be found at www.pfizer.com.au/privacy. As Pfizer is part of a global group of companies some of your information may be transferred to servers located in countries other than Australia including the United States and Singapore for storage and adverse event collection purposes.

Your personal and sensitive information will be collected by a third party service provider on behalf of Pfizer and may be disclosed to other third parties involved in administering and hosting the Program. WAX Interactive Pty Ltd currently administers the Program and the information contained therein and will handle your personal and health information in accordance with this Program Privacy Statement, applicable laws (including the Privacy Act 1988 (Cth)) and Pfizer’s Privacy Policy.

If you opt out of this program, your personal and sensitive information will only be retained for record keeping and auditing purposes and as required by law or regulation. If this Program is terminated, your personal and sensitive information will only be retained for record keeping and auditing purposes and as required by law or regulation.

By providing your personal and sensitive information as part of the Program, you consent to the collection, storage, use and disclosure of your personal and sensitive information as set out above.
